Q: Who may I enroll under the HPRS dental and vision coverage?
A:A retiree may enroll a spouse and any eligible dependent children until age 26. Dependent children are stepchildren, natural children, adopted children, or children for whom the retirant is the legal guardian. Children for whom the retirant is the legal guardian are eligible until age 18. There are eligibility restrictions for spouses and children. Please contact HPRS prior to retirement for further details.
Q: When may I enroll or terminate dependent coverage for medical, dental, and
vision coverage?
A:Open enrollment for dental and vision coverage is held annually in November with the effective date of coverage the following January 1st. If you do not elect to enroll yourself or eligible dependents at retirement, you must wait until the next open enrollment period.
Dental and vision coverage can only be terminated during open enrollment, due to a qualifying event, or when a covered dependent no longer meets eligibility requirements.
The effective date of coverage will be:
- January 1st for an addition during open enrollment. Terminations during open enrollment shall be effective December 31st.
- The first day of the month following the receipt of an enrollment form based on a qualifying event.
- The date of marriage for the addition of a new spouse or stepchild.
- The date of birth for the addition of a newborn.
- The adoption date for the addition of a newly-adopted child or the date the child is placed for adoption.
- The date the legal guardianship becomes effective.
Q: What is a qualifying event?
A:Qualifying events include: (documentation is required)
- Marriage,
- Birth, adoption, placement for adoption or legal guardianship of a child,
- Change in employment status,
- Legal separation,
- Involuntary termination of other group coverage, or
- Death.
